Medicare's 2027 Home Health Payment Proposal: What It Means for Aides
Medicare has proposed paying home health agencies 2.4 percent more in 2027, after a cut this year. What that does and does not mean for aides.
On July 1, the Centers for Medicare and Medicaid Services put out its proposed home health payment rule for 2027. This is the rule that decides how much Medicare pays the agencies that hire home health aides. Comments closed on August 31.
It is not a rule about your paycheck. I still think you should know about it.
What they proposed
CMS expects Medicare payments to home health agencies to go up by about 2.4 percent in 2027 compared with this year. That is roughly 420 million dollars across the country. It comes after a cut. The final rule for 2026, which came out on November 28, 2025, was expected to lower payments to agencies by about 1.3 percent, or around 220 million dollars.
Two smaller parts caught my eye. CMS says it will give examples of palliative care that fits under the Medicare home health benefit, through guidance rather than a new regulation. It also asked the public whether home health should get its own wage index instead of the hospital one it uses today.

Why this matters to an aide
A lot of home health agencies depend on Medicare money. When those payments shrink, agencies get careful about hours, hiring and how many visits they can staff. When they grow, there is a little more room. The rule does not tell any agency what to pay you, and a 2.4 percent rise in total payments is not a 2.4 percent raise.
The wage index question is the one I would keep an eye on. It is how Medicare adjusts payments for what labor costs where you live. A version built from home health data could push payments up in some places and down in others. For now CMS is only asking.
What stays the same
Your training and your exam. The federal rules for aide training at 42 CFR 484.80 do not change. You still need at least 75 hours of training, 16 of them hands on under supervision, and then a competency evaluation.
What comes next
CMS reads the comments and then publishes a final rule. Last year that happened at the end of November. I will post again when the 2027 version is out.
